Instructions For Application

• Right-Click on the Green Button below.
• A Menu will appear.
  
• Click on Save Link As, or Save Target As, depending on the browser you are using.
  
• Save the Application to your computer.
  
• Preferably the desktop, so you can easily locate it.
• Locate the Application, and double-click it.
  
• It should open with Acrobat Reader.
  
• Fill it out, and click the Submit button.
  
• If your computer does not have Acrobat Reader,
  click the button below to download it.
